About TMS Therapy

Transcranial Magnetic Stimulation Therapy (TMS) is a form of neuromodulation that is uniquely suited to treat patients with TRD (treatment-resistant depression), MDD (major depressive disorder), and OCD (obsessive-compulsive disorder) and has higher response rates than antidepressant medications alone. 

TMS can be used as a stand-alone treatment, or in conjunction with a patient’s existing medication and therapy. At Achieve TMS, we always encourage our patients to use TMS as one part of a multi-dimensional mental health plan. TMS is a particularly helpful option for patients who cannot tolerate the side effects of medication, or are not getting sufficient relief from medication and/or therapy.

TMS for Depression

For treatment of MDD (major depressive disorder) or TRD (treatment-resistant depression), TMS uses an MRI-strength magnetic coil to send magnetic pulses to the dorsolateral prefrontal cortex, causing depolarization of neurons that fMRI’s have shown to be underactive in patients with depression in comparison to healthy brains. As these neurons become more active over the course of a 6 week TMS treatment plan, most patients experience better quality of sleep, relief from depression symptoms, and improved ability to regulate mood and handle emotional stress. TMS has a three-week latency period, when the effects of treatment are most notable, and delivers long-lasting results that often give patients years of symptom relief. ( More Information Below)

TMS for Obsessive-Compulsive Disorder

For treatment of OCD, TMS sends magnetic pulses to the anterior cingulate cortex, as a means of stimulating the Papez circuit and interrupting the neural loop that traps patients in repetitive thought and compulsion sequences. As these neural circuits become interrupted over the course of a 4 to 6 week TMS treatment plan, these obsessive thoughts and compulsive behaviors become less and less pronounced and occur with decreasing frequency.

Providers should note that TMS was FDA-cleared to treat OCD in 2018, and most major insurance carriers are still considering coverage on a single case agreement basis. Our insurance specialists are working diligently to expand coverage and advocate on behalf of our patients.

Typical Course of Treatment

Although treatment varies by individual and insurance coverage, a typical treatment course consists of:

  • 30–36 treatment sessions
  • Conducted over a 6 week period
  • 5 treatments per week
  • Each treatment session lasts approximately 20 minutes
  • Treatment is not painful, and patients watch television or read during each session
  • Patients can immediately drive, work, and resume everyday activities after each daily treatment

  • Minimal Side Effects – No drug-related side effects such as weight gain, drowsiness, nausea, insomnia, or decreased libido. The only side-effect is mild headache, which typically resolves after the first week of treatment, and can be mitigated with an over-the-counter pain reliever
